Good morning! Good morning!

Cold enough for you? Not that this was ever funny but it sure isn’t right now. The temperature (and wind chill) are facts. You can measure them accurately using a standard scale and get no argument when you report the number.  The key to how cold you feel is the degree to which you prepare. When “Winter camping” we bring clothes made for cold temperatures and plenty of them. We bring boots and sleeping bags rated for cold temperatures and plan meals and activities to keep our body temperature comfortable. “So how does this apply to pharmaceuticals ?” you might ask. (or “Has the cold affected his brain?” might be the other question.) 

Well we see the pharmaceutical forecasts (High cost injectable pharmaceutical on the rise with a huge high pressure area coming in from the retired members, A blizzard of political rhetoric on the horizon accompanied by a lot of hot air.)

If you haven’t been planning for this storm you may still have time. Evaluate your prior authorization program, institute mandatory generics and a stepped approach to using high cost drugs.  Prepare for the high cost injectables. Your highest users (the 5% who use 50% of your assets) may benefit from closer scrutiny of all of their drugs and some may benefit from care management.  

I doubt that the warming of the US economy will obviate the need to plan and then take action. Members are lined up at the pharmaceutical ticket counter expecting you to pay 80% of their pharmaceutical trip to a warm comfortable place.  Use the PBM tools you have to prepare your plan for the future storms you know your company will have to weather in order to remain viable. You can plan ….or get lost in a hot toddy by the fireside while looking at the want ads.

 

1)  21…You win!
 

Not a card game but the FDA going back to the way they used to keep score. Congress would always ask the FDA how many new drugs were approved every year. The expectation was that MORE were better. At one time they kept approving "me too" drugs….an existing chemical with a slight change to make it patentable but not significantly different from the original. This year there were quite a few significantly different drugs for cancer, HIV and  infections. Over 320 generic products were given the thumbs up as well. The big news is in the BIOLOGICALS area…72 new drugs….. and all with high price tags. 2)  Sorry gramps, but I need my own health coverage…
   10 percent of large employers dropped retiree health coverage in 2003. Expect more to bail as the new MEDICARE benefits hit. Government incentives won't help, and unions may pay lip service but likely to want to protect their own health benefits at the expense of their former co workers. Beth Steel can be the poster child. They had 6 retirees for every 1 worker. You don't think that contributed to the end of the company and the active jobs do you? http://www.washingtonpost.com/wp-dyn/articles/A18168-2004Jan14.html

Another big push for UNIVERSAL HEALTH COVERAGE, "Universal" probably only for the 43 Million UNINSURED. Health care providers will not want to treat their regular paying customers at government rates…..Docs and all will get behind a chance to treat more patients but not at they expense of what they already have…..just human nature. http://www.washingtonpost.com/wp-dyn/articles/A18404-2004Jan14.html

5)  Generic CIPRO held off for another 6 months due to kiddies…
  The "pediatric card" was played by the originator. FDA will grant another 6 months of exclusivity to market a drug if the originator does drug studies in kids. I am not surprised from an economic standpoint but this drug has bee show to cause spontaneous rupture of the Achilles tendon and CONTRAINDICATED in kids. I suppose if it worked for ORAL CONTRACEPTIVES and this you could see a pediatric extension for Viagra in a few years…..http://biz.yahoo.com/rc/031218/health_bayer_cipro_1.html

6) Mixed news for autism….
  Study of new drug aimed at autism does not find all the answers. Appears the drug works in less severe forms but not the worst cases. I would like to see them follow up and treat this like the Alzheimer's drugs….some relief in some patients is better that no relief for anyone… time will tell. 7) Lassie won't rescue little Timmy this time….
 

FDA say no to Governor Tim Pawlenty's   illegal bid to import Canadian drugs. May have to send him two e-mails: one reading "N" and the second reading "O" for the message to get through. I am surprised that Minnesota Pharmacists have not beaten his door down for the business that they will lose. (Maybe it is because of the pharmacist shortage that they can't afford the time to chase him down). http://www.twincities.com/mld/pioneerpress/news/politics/7722842.htm
