Pepin’s Pharmaceutical Prattle for 02-14-2005

Happy St. Valentine’s Day

 However we got where we are today, make the most of this celebration of love.  Pick a single Valentine. (It always bothered me in grade school to have to write out 50 little Valentines to make sure no other student would feel left out…. Valentine was, to me, meant to be singular! ). Tell that special someone how you feel. Don’t just buy a big frilly card with verse already in place, add a comma and your name!  Find a frilly, but blank, card and start to write. I’ll give you a framework if you are lacking structure. Remember the word ACTS and what it stands for:

1. Adoration- Tell your love of your admiration and devotion.

2. Contrition- Beg pardon for any previous missteps or inattentiveness

3. Thanksgiving- Express your thanks for the love, companionship and time you have been able to celebrate with each other

4. Supplication- Ask for the return of your love and affection for eternity.

Pretty simple. You may wish to embellish each of the four areas with examples but I would suggest that you be brief and a bit vague on the second point.

When you are satisfied with your work on scratch paper, find a good pen and write it out longhand. (Do not e-mail or include in typed fashion no matter how bad your handwriting.) Now comes the most important part…. Even if you had writers block or couldn’t find a blank card and need to use a “store bought” card be sure to write out the words I love you before signing the card.  (Do this even if “I love you” is already on the card).

 You can add a gift, flowers, or candy but I’ll bet that this special Valentine will be remembered long after the flowers wilt or the pounds come off.

President Bush proposes more funds for the FDA to do post marking survelience…..You don’t think this has anything to do with the Vioxx recall do you?

2) You could die, A?

Do I have your attention? Canadian government halted the (legitimate) sale of Adderall XR (for attention deficit disorder…. Pay attention!)  due to “sudden death”. The FDA has now issued a HEALTH ADVISORY. It might make parents feel better to forego the Adderall this weekend…. it would make me crazy with worry to take any chances with my kids. The drug is still available here in “the States” but simply watching for signs of “sudden death” would seem silly…. If there were “signs” then would it really be sudden? Where is George Carlan when you need him? This warning could escalate so you may wish to bookmark both websites.

3) Speaking of health risks/prevention in kids…

It is advisable, per the CDC advisory panel, to have your children vaccinated against meningitis. Suggestion is to vaccinate all 11 and 12 year olds. The vaccine last for 8 years and prevents people from being carriers. The vaccine should be available in March, 2005. Whatever you do, don’t send the kids off to college without it. The pharmacoeconomic studies don’t show good numbers but the panel and parents don’t care. This vaccine won’t save money…just lives.

4) OnStar?... Give Mrs Jones a refill of her fluoxetine!

Big three automakers band together to make e-prescribing a reality in Michigan. The technology has yet to catch fire nationally although it is included in the new Medicare Part D regulations. I hope this, and RxHub spark more physicians to embrace this way of prescribing.

5) What’s your octane?

This article speaks to increasing fixed dollar copayments in tiered drug plans over time. Imagine if you could buy all the gasoline you could burn for $0.40/ gallon, about 20% of the actual cost. Imagine how fast and how far you’d go. The reality is that the cost of “gas” (prescription drugs) has been going up. The big reality hits retirees who now have to pay full price when they retire or leave a company spnsored pharmacy benefit plan. They are outraged! (Welcome to the world of employers!). In truth, the tier approach is just asking everyone to pay a little more for “Premium”, than for “Silver” than for “Regular”. Your body and your doctor know what “octane” you need and everyone knows it is silly to put “Premium” into a car that runs perfectly well on “Regular”. I hope the new Medcare Part-D benefit uses tiered benefits (see item #7, below,for the price-tag)!

6) The pills made me do it!

FDA changes its website warning on antidepressants, http://www.fda.gov/cder/drug/antidepressants/default.htm , to use  "(antidepressants) increased the risk of suicidal thinking and behavior in short-term studies of adolescents and children"  instead of previous language that revolved around cause. Takes some of the wind out of “the drugs made me kill them” defense used by a (then) 12year old who killed his grandparents.

7) A Billion here… a Billion there…pretty soon we are talking some real money!

Medicare drug benefit estimates are coming in even higher that the Democrats said they would. Should be pretty hard for the AARP to complain that they aren’t getting enough! (“Pretty hard” but I doubt “impossible”).
