Pepin’s Pharmaceutical Prattle for 03-01-2004

Quote of the day:

Self-respect is the fruit of discipline; the sense of dignity grows with the ability to say no to oneself.   Abraham J. Heschel

Good morning! Good morning!

Happy Lent!  Ever wonder why some people "give up" things for Lent? In short it ties into todays' quote. If we voluntarily say "no" to good things we like (Ice cream, sweets, beer, movies, TV, meat, etc) then maybe it will build our character to the point where we can say "no" to the things that are not good for us spiritually (certain movies, swearing, _________, and __________ [you fill in the blanks]). Try to "give up" something good that you like as a test for yourself. If you fail then there is no harm done. Next you might try to give up bad habits…. here you may benefit physically and spiritually.

A test of 40 days may be just the ticket for becoming the "better person" that we all long to be. It is all too easy to pick out thing for other people…. as you can see there are politicians that I would like to see "give up" their crazy positions, a few lawyers would could stop chasing ambulances, etc.

In addition to the "negative" aspects of "giving something up" you could always choose the positive approach. Be nicer, work harder, give more to charity, pray…. This might do even more good. You know yourself…. maybe a little of each.  It couldn't hurt!

1) "Senior moments" about Medicare drug benefit…

Confusion about the MEDICARE drug benefit makes it a ripe issue for DemocRATS and RepublICANs. (If they can argue about possible subliminal messages on a political add then you can bet they'll argue this one into the ground).{You do remember the RATS issue from a few years ago, don't you?)

2) You are what you drink?

Must be a slow research day when British "researchers" resort this. They searched the literature for any studies of whether "drink plenty of fluids" was ever studied as a "cold" treatment. Finding none they bring up a few conditions in which excess fluid intake could pose a health hazard. While this may be a case of "evidence based medicine" asking for data, I'll still drink extra water when I have the sniffles. The other advice they give about taking antihistamines and acetaminophen or more suspect than a glass of water!

  3) If you haven't used your prescription after 1 year…maybe you should have.

Walgreens is being sued for placing expiration dates on prescriptions. "DO NOT USE AFTER…." is required by most states. One year after dispensing is the common time frame. The plaintiff believes that expiration dates are placed on the prescriptions in order to sell more drugs instead of for safety/legal reasons. If you still have some pills left from a 30 day prescription that you had filled 1 year ago then either you are not compliant (in a MAJOR way) or the "condition" does not exist anymore. All good advisors urge people to throw away anything that they are not using. Don't expect this to go very far in the courts. (Maybe the drugs concerned were anti anxiety agents or antipsychotics and compliance should be strongly encouraged!)

  4) Don't expect to see a law-firm called McCain, Dorgan, and McClellan.

Two senators say they will hold up the nomination of McClellan to head CMS because he enforces the laws against "re-importation" of drugs from Canada. Both of the Senators are bright enough to "get it" but I'll try to put it in terms that their staff might understand.

One- It is illegal

Two- It is LESS safe than getting drugs manufactured, distributed and labeled for use in the USA.

Three- price controls in Canada make the drugs less expensive for Canadians but work against the economics of the pharmaceutical business in the USA.

If these good Senators had each had a business that sold WIDGETS they might understand the economics better. If they sold 5% of their widgets to Canada for less than they sell them for in the US due to price controls but still made a small amount above production then everyone could be happy. If those widgets were sold back into the US and undercut the US price it would cause a 5% decrease in the US market for WIDGETS. So far, not too bad. As demand for cheap widgets rises then the Canadians would order more and more widgets so that they could sell back into the US and further undermine the US market. Pharmaceutical companies such as GlaxoSmithKline,, Lilly and Pfizer recognize this and are limiting the amount of drug that they will sell to Canada.

If the demagogues were honest in their approach to pharmaceuticals then they would propose one of two "solutions" (of which I subscribe to neither):

1) Nationalization of the US pharmaceutical industry

(like the government could run it more efficiently… right!)

2) strict price controls on pharmaceuticals

(and then how about college tuition and automobiles and….)

Short of either of these "solutions" they should stop hiding behind Canadian price controls. (Just my opinion…I could be wrong… but I'll bet I'm not!)

5) New drug- expensive and intravenous-

Avastin (bevacizumab) was approved for colon cancer treatment. In the past I would not have commented on such treatment because most high cost injectables were covered under the medical plan and not the pharmacy benefit. Now these drugs are being pushed out by the medical providers (or they'll supply them for AWP PLUS about 25%) … better make plans for this…. you are going to end up paying…. one way or the other.

6) MN head is lost
The "Gov" is threatening to sell off the Pfizer stock held by the state in pension funds. If he succeeds in his crazy plans to import Canadian drugs then it is in the best interest of the pensioners to sell now.

ps: If the Govenor want to save some REAL money then he should call me for a consultation...... like THAT is going to happen!

7) Walter finds work….

Cronkite set to be the Mike Ditka for opponents of the recent Medicare law. I am surprised to find that with a successful broadcast career and at 87 years of age that he needs the money. I guess it is better than becoming a Walmart Greeter....

Have a SUPER-FANTASTIC week. Steve
